Include verification of BA/BS or higher degree (college/univ transcripts) and License/Certificate issued by California Board of Occupational Therapy when submitting completed application form. District application available by clicking on link (or accessing through BPSD website). Not fillable on line. Please print out, complete and submit via mail or in person. Thank you.
Note: District office closed in observance of holidays on December 24, 25, 26, 31 and January 1 and 2. Offices will reopen at 8am on Monday, January 5, 2015.
