  • CPR/First Aid Certification (Possess a valid CPR Certificate (Must include Sudden Cardiac Arrest.) and possess a valid First Aid Certificate)
  • Other (Copy of Concussion in Sports Certification. www.nfhslearn.com, Heat-related Illness Certification, and Sudden Cardiac Arrest Certification. )
  • Resume (Current Resume)
  • TB Screening Result (Valid TB verification)
