Requirements / Qualifications
BASIC FUNCTION: Under the direction of an assigned supervisor, the Community Health Outreach Worker (CHOW) is responsible for outreach to both the school and the community at large to promote the Wellness Program and its services. This outreach includes promotion of the Wellness Program to students, school staff, parents, and Community-Based Organizations (CBOs). The CHOW also serves as a member of the Wellness Program interdisciplinary team. The CHOW establishes partnerships with CBOs and coordinates services to meet the needs of the school community. KNOWLEDGE AND ABILITIES: Establish and maintain collaborative partnerships with Community Based Organizations (CBOs) Triage Referrals Provide linkage to and follow-up with students/families on referrals to onsite and outside Community Based Agencies Organize support groups, outreach, and educational workshops in collaboration with onsite credentialed staff member Collaborate on planning, coordinating, and implementing the Wellness Program Complete documentation and assist with data-collection for the Wellness Program
PHYSICAL ABILITIES: Vision to monitor students during classroom activities, hearing and speaking to exchange information and provide assistance to students, bending at the waist, walking and standing for extended periods of time, and dexterity of hands and fingers to operate specialized equipment or office equipment. LICENSES AND OTHER REQUIREMENTS: Valid California Drivers License as this position may travel between and serve multiple school sites EDUCATION AND EXPERIENCE: This position requires the candidate to have completed at least two years of study at an institution of higher education. It is preferred that the candidate have a college degree and experience working in an educational environment or a community based organization. Candidate must have knowledge of principles and practices relating to mental health, community based organization and learning environments. Correct English usage, grammar, spelling, punctuation and vocabulary, as well as good oral and written communication skills are essential.
