Under the direction of an assigned supervisor, provide case management duties to assure the delivery of services for children with behavioral, academic, social or emotional problems caused by a variety of physical, social, and psychological factors; assure proper linkage of children and their families to various social, economic and community resources.
REPRESENTATIVE DUTIES:
ESSENTIAL DUTIES:
Receive referrals and collect data from family, community, or school district records. Interview clients during the intake process and document information used to complete required forms. Make referrals to community agencies and provide follow up services to monitor progress toward stated objectives. Provide interventions to assist child in improving, maintaining, or restoring functional, educational, social and leisure skills. Assist in coordinating services at the onset of a crisis to provide support and assistance in problem resolution; arrange for provision of other services as required. Provide support and information to family members and significant others to increase the probability of positive change; monitor and address problem areas. Attend staff meetings, request assistance and recommendations regarding complex situations and case management issues. Maintain case management charting; monitor services provided to assure that child's objectives are met. Attend a variety of meetings and conference related to case management; maintain interagency contracts. Monitor child's progress through periodic contacts with family, school, and community agencies as appropriate. Perform related duties as assigned.

Any combination equivalent to a two-year college degree in business, public administration, psychology, sociology, education or other closely related field and 2 years qualifying experience in human services field.
