Job Summary: Provides support and establishes a trusting relationship between students, families, school, and agency personnel. This is a specially funded position dependent upon funding for continuance. Minimum Qualifications & Abilities: High school diploma or equivalent. BA/BS in Education, Behavioral Services, Human Services, Social Services, or equivalent preferred. Must obtain and maintain a Certificate of Completion for Chronic-Neglect Parenting Curriculum Training. Two (2) years of experience in a public agency, government-funded or similar program; experience in a health, education, or social services related field preferred. Ability to understand and follow oral and written directions. Ability to work effectively with staff, students, parents, and the public. Knowledge of community agencies and resources. Ability to work with individuals of diverse backgrounds. Ability to communicate effectively in oral and written form in both English and Spanish at a professional level. Valid California Driver’s License with appropriate DMV printout. Personal car for use on district business with appropriate insurance and ability to transport clients as needed.

Job Description / Essential Elements:    Print   

Essential Functions: 

  • Compiles community resources and referral information.
  • Assists in scheduling and facilitating Family Resource Center community and governing meetings.
  • Provides general support to the Family Advocates.
  • Processes referrals from staff, parents, and community.
  • Reviews family service plan (with input from students/families) developed by school, agency and/or intervention teams.
  • Conducts home visits and completes assessments to identify family strengths, areas of need and resources.
  • Using case management techniques, assists and advocates for families in implementing service plans by facilitating access to and “linkages” with appropriate agencies and resources.
  • Develops service plans when appropriate.
  • Maintains effective communications regarding activities and events between and among all partners.
  • Maintains an accurate record of all Initiative activities and events.
  • Coordinates additional needs assessments or resource mapping as assigned.
  • Coordinates a comprehensive training program for Initiative participants and staff, including court mandated parenting classes.
  • Assists in the organization of the annual Community Health Fair.
  • Assists in the development and implementation of Kindergarten Bridging Experience and Kindergarten Transition Orientation for preschool children and their parents.
  • Assists in the development and implementation of Outreach Programs for preschool and child care providers.
  • Assists with the articulation meetings for preschool providers and kindergarten teachers.
  • Attends essential trainings per grant requirements.
  • Processes paperwork, prepares reports, and maintains files as necessary.
  • Performs other duties as assigned by Superintendent or designee.
