  1. Credential
    1. Single Subject: Chemistry and Biological Sciences Credential or equivalent. Please upload the "Document's Detail" of your credential. For instructions on how to obtain a detailed document see "Print a Document's Detail" by clicking here.
    2. If Intern eligible, upload an Intern Eligibility Letter in both subject areas. Intern applicants must include exam results and/or proof of registration for exams (i.e. CSET, RICA, TPA).
    3. Applicants must have one credential authorization and be willing to go on a plan to complete the other within a set period of time. This plan includes being eligible to apply/renew a CTC emergency document which may include passing exam(s) and/or progress toward completion of coursework.
  2. Transcripts
    1. Please submit official transcripts showing completion of a baccalaureate or higher degree from an accredited college or university.
    2. Out of country transcripts may be submitted along with an evaluation from an agency approved by the Commission on Teacher Credentialing. For a list of approved evaluating agencies please, click here.
  3. Letter of Introduction

  5. Letters of Recommendation
    1. Three letters of recommendation signed and dated within the last 3 years.
    2. Acceptable signatures include original wet signatures, Adobe or DocuSign electronic signatures.
