General Description
Under the direction of the Director of Student Health Center, the Mental Health Case Coordinator provides nonclinical social work services, including initial scheduling of intakes, student advocacy, information and referral coordination of on-and off-campus wellness resources.

The Mental Health Case Coordinator improves student success by ensuring responsive, culturally-sensitive, support services to students with immediate needs and coordinates with community organizations to obtain resources to promote student success, retention, and completion by removing barriers to higher education.

Distinguishing Characteristics:

The Mental Health Case Coordinator position requires the ability to work independently while developing strong collaborative relationships with colleagues, students, student services, academic service units, and community partners.

This is a full-time, 10-month position. M-F 7:45-4:45pm. Initial placement is at $33.39 hr.This position is grant funded and will end when funding for this position ends.

Job Duties
Job duties of the Mental Health Case Coordinator:

• Serve as the centralized point-of-contact to facilitate students' access to mental health services, campus and community-based resources. Assist students in navigating college support services and referring to the appropriate on- and off-campus services as needed.
• Review mental health counseling intakes with the purpose of scheduling students for mental health counseling, triaging, directing immediate needs to appropriate providers.
• Conduct individualized, equitable risk/need assessments for students in need. Document progress, goals and any other information-requiring follow-up.
• Maintain accurate and confidential records of every student interaction including in-person visits, phone, and online consultations.
• Coordination with on campus and community resources to ensure students have access to mental health and basic needs services. Serve as a point of contact for faculty/staff who identify a student in crisis.
• Collaborate with various campus offices and community organizations to develop resource list of services for students
• Refer students to basic needs resources, to receive CalFresh, Medi-Cal, Covered California and other public benefits.
• Work with mental health providers to plan, coordinate and implement mental health activities/events on campus.
• Collaborate with basic needs coordinator for referrals to basic needs and to mental health counseling. Maintain monthly data on student's enrollment/participation in services for grant funded programs/resources. Ability to be trained to use EHR software.
• Utilize the District's integrated software in performing the required duties of the position.
• Perform other duties as assigned.

Qualifications
Minimum Qualifications:

Any combination equivalent to: Bachelor's degree in Social Work or related field from an accredited institution and undergrad fieldwork or professional experience related to the area of assignment.

• Travel to various sites to conduct duties of position, including but not limited to the Hanford Center and Tulare Center.
• Extensive contact with students
• Multi-project, fast-paced working environment.
• Some weekend and evening hours may be required

Physical Abilities Ability to:

• Operate a computer and office equipment
• Sit and stand for extended periods of time.
• Communicate with students/employees/public in writing, by e-mail, in person or on the telephone.
• Read and write various materials.

Knowledge and Abilities
Knowledge of:

• Principles and practices of planning, case management, organization and coordination of mental health care and other supports.
• Crisis intervention skills.
• Principles and practices of leadership, advocacy, collaboration and customer service.
• Needs, interests, issues and concerns of various groups of community college students.
• COS Student Services and applicable local community services.
• Applicable federal, state and local laws, rules and regulations, including the Family Educational Rights and Privacy Act.
• Communication skills in both one-on-one and group settings.
• Analytical and critical thinking skills.
• Interpersonal skills using tact, patience and courtesy.
• MS Suite to include Word and Excel.

Ability to:

• Provide information and guidance to students related to mental health services and supports. Provide leadership, and support to staff involved in the mental health counseling program.
• Communicate clearly in both oral and written form and follow oral and written instructions.
• Work with under-represented populations and the ability to work successfully with students of diverse academic, socio-economic, ethnic, cultural, and disability backgrounds. Interpret, apply, and explain policies, procedures and regulations regarding the mental health counseling and basic needs programs.
• Work independently while developing strong collaborative relationships with colleagues, students, student services, academic service units and community agencies.
• Organize set priorities and exercise expert, independent judgment within areas of responsibility; manage multiple work priorities.
• Represent the District effectively in dealings with students, community groups and the public
• Make presentations and present proposals and recommendations clearly, logically and persuasively to diverse audiences
