- Certificate Copy (School Bus Driver Certificate)
- Certification (T-01 Card)
- CPR/First Aid Certification (First Aid - OR - no restrictions on line item 6 of the School Bus Driver Certificate)
- DMV Printout (Less than 30 days old)
- Driver's License Copy (Commercial Driver License with P/S endorsements)
- Other (DL-51 (DMV medical fitness form))
